Forms and Special Elective Information

Please Read Before Downloading Visiting Student Form!

Students from institutions accredited by the Liaison Committee on Medical Education (LCME) interested in applying for elective rotations at Robert Wood Johnson Medical School in Camden, NJ and our affiliate hospital, Cooper University Hospital, must supply the following documentation to the Registrar's Office:

• Complete Visiting Student Application
• Complete HIPAA Training form
• Complete and up-dated Visiting Students Immune Status form
• Proof of Personal Heath Insurance coverage
• A copy of your Criminal Background Check

All application information must be received at least six weeks prior to the start of the elective rotation. Please do not directly submit application materials to academic departments.
